Registration Forms

In order for your child to be placed on the ACTIVE roster you will need to complete the following forms (available at sign ups):

  • Player Package (works only with internet explorer) which includes the following:
    • Registration Form
    • Waiver Form
    • Sports Physical
  • A copy of your Insurance Card
  • A copy of your Birth Certificate
  • A copy of your 2010 - 2011 Report Card
  • Code of Conduct signed by Parent and Child
  • All fees paid in full
  • Texas Roadhouse tickets (available at sign ups)

Once you make a payment, bring a copy of your receipt and all the necessary forms to the next sign up so that we can place your child on a team. If you did not pay in full then you will receive the fund raising information at that time.

Payment

There is a $5 convenience fee to pay on-line.

 

Definition of 'Complete Registration/Fundraising'
Choose this option to register your child and pay all money for the year.
You will still need to fill out the following forms to register your child, but the money will be paid and no fund raising will need to take place.

Once you add an item to the Cart your cart will be updated in the upper right hand corner of the screen.

Once you have completed adding things to your cart you need to hit the Google Checkout Button to complete your purchase

This will take you to another site - you must fill that out to complete the payment process of your Registration - print a copy of your receipt.
